Off-Site Health Screening Appointments

2025 Off-site State-sponsored Health Screenings are now provided by eHealthScreenings (a Premise Health company).

Note: An off-site health screening event is one that does not take place at a Montana Health Center. An on-site screening event is a health screening that takes place at a Montana Health Center location in Anaconda, Billings, Butte, Helena, or Missoula. Appointments for an on-site health screening can be made at mypremisehealth.com

 

Make an Off-site Health Screening Appointment

All members will need to register for an account. 

Account Registration

  1. Go to ehealthscreenings.com/signup.
  2. Under "Not Yet Registered?" enter screening key EPI141 then click "Submit."
  3. On the Verify Account page, your temporary user name and password have been assigned and are case sensitive. Once entered click "Verify My Account".  
    • Username: First Initial + Last Initial + DOB (MMDDYYYY) (example: John Smith born 4/23/1975 is JS04231975)
    • Password: Last Name + Last 4 SSN (example: John Smith with social 123456789 is Smith6789)
    • For participants with hyphenated last names, use the first letter of the first last name.  For example, Jane Myers-Smith was born 4/23/1975 and her SSN is 123456789.
      • Username: JM04231975, Password: Myers6789
  4. Update account information and follow the prompts. 

 

Scheduling an Appointment

  1. Login to your account at ehealthscreenings.com
  2. Enter screening key EPI141
  3. Select the location, date and time then click 'register'
  4. Follow the prompts
    • You will be prompted to select from three optional tests. Do not add these tests unless your provider has asked you to do so.

 

Change or Cancel an Appointment

  1. Click the "Reschedule/Cancel" button in your appointment confirmation email OR login to your account at ehealthscreenings.com.
  2. Under the calendar icon that shows your upcoming appoint, click "click to select"
  3. Choose the appointment you want to change, and click on "Reschedule" or "Cancel."
  4. Follow the prompts.

 

Important Reminders

  • If you cannot attend your scheduled appointment for any reason, cancel your appointment as soon as you know you won’t be able to make it.
  • Do not schedule multiple appointments.
  • Spouses and domestic partners must have their own health screening appointment.
  • Don't attend your appointment if you are not feeling well. 

 

Access Results

Health screening results take about 7 days to appear on the eHealthScreening portal.
  1. Login to your account at ehealthscreenings.com
  2. Under Step 3: "View Current Results," click on "Click to Select" on the homepage.
  3. On your Result Dashboard, click "Print Reports" at top of the page.
  4. Select the report you'd like to print. 
    • Once you select on a report it takes about 30 - 45 seconds to generate depending on your internet speed.
