CareHere is now Premise Health! 

What does that mean for you as a member?  The CareHere patient portal and app will no longer be available for appointment scheduling effective 04/27/2021. Instead, members can make appointments at mypremisehealth.com, by downloading the My Premise Health app from the Apple or Google Play App stores, or by calling (855) 200-6822.

Historical patient data from CareHere is accessible by you and your care team through March 2022 by using the CareHere website or app. If you wish to receive a full copy of your medical records, call (855) 200-6822.

Premise Health DOES NOT accept walk-in appointments.  Please go to mypremisehealth.com or call (855) 200-6822 to schedule an appointment.

3 Options to Schedule Your Appointment

Using the MyPremiseHealth App:
Download the My Premise Health app in the Apple or Google Play store.
By Phone:
Call Premise Health at (855) 200-6822 for all health center locations.
Online:
Go to mypremisehealth.com and sign in. First time?  See the Registration instructions below.

 

How to Register on My Premise Health

  1. Go to mypremisehealth.com and click “SIGN UP NOW”.
  2. Follow the prompts to setup your account.
  3. If you cover a dependent under 18 on your plan, you must request access to their chart in order to make appointments for them.
    • Use your My Premise Health account, log in, click “Menu” in the top left-hand corner, select “Request Minor Chart Access,” enter the requested information and click “Submit.”  This can take up to one business day to complete; or
    • Call (855) 200-6822 and schedule the appointment for your dependent.  When you arrive for your appointment, request portal access for your dependent(s).
  4. A spouse, domestic partner, or dependent 18 or older on your plan must create their own account and make their own appointments.   
  5. If you need assistance with registration visit mypremisehealth.com and click "Contact Support" under the "Need Help" section.

How to Schedule an Appointment

  1. Once you are registered, login to My Premise Health.  If prompted, choose yourself or a dependent under 18 you have previously registered (number 3 under "How to Register..." above).
  2. Click “Schedule an Appointment” at the top of the page.
  3. Select the appropriate tile that pertains to your visit under “Tell Us Why You’re Coming In.” When you schedule an appointment, the new system will automatically adjust the length of your appointment time based on the reason for and complexity of your visit.    If you are unable to find the appointment type you need for scheduling, contact the health center for assistance.
    • For Common Medical Appointments, click here.
    • For Flu Shots: Starting January 1, 2022, please call the Health Center for a flu shot appointment at (855) 200-6822.
    • For Health Screenings at a Montana Health Center: Select “Biometric Screening.” Please do NOT make an appointment on a Provider's schedule for a health screening.  You will be entered to win a $50 gift card if you complete a health screening by August 31!
    • For Health Screenings at an off-site event: Select “Biometric Screening Mass Event.” You will be entered to win a $50 gift card if you complete a health screening by August 31! 
    • For Wellness Coaching (Health Coaching): Select the coaching option that best fits your needs. 
    • For Premise Health Group Presentations: See "How To Register for a Premise Health Group Presentation," below.
  4. Answer the COVID-19 screening questions.
  5. Select the location.
  6. Select your provider.
    • For the Helena Health Center health screenings and lab appointments and for off-site screening events, select your phlebotomist. For all other health center locations, select the generic lab schedule.
  7. Select the date and time of your appointment.
  8. Verify your appointment details and click “Schedule.”
  9. When you schedule an appointment, this appointment is for you.  A spouse or dependent need to have their own appointment.

How to schedule a Same Day Appointment

  1. Once you are registered, login to My Premise Health.  If prompted, choose yourself or a dependent under 18 you have previously registered (number 3 under "How to Register..." above).
  2. Click “Schedule an Appointment” at the top of the page.
  3. Select the appropriate tile that pertains to your visit under “Tell Us Why You’re Coming In.” When you schedule an appointment, the new system will automatically adjust the length of your appointment time based on the reason for and complexity of your visit.    If you are unable to find the appointment type you need for scheduling, contact the health center for assistance.
  4. Select "Helena Montana Helath Center"
  5. Select "Helena Same Day Provider"
  6. If an appointment is still available for Same Day, select the time you want.
  7. If the appointment time is correct, click "This information is correct"
  8. Click "Schedule"

How To Register for a Premise Health Group Presentation

  1. Once you have registered with My Premise Health, login to your account.
  2. Click “Schedule an Appointment” at the top of the page.
  3. Select the appropriate tile (Lifestyle Seminar, Physical Activity Seminar, or Nutrition Seminar) under “Tell Us Why You’re Coming In.” You can find this information in the class advertisement.
  4. Answer the COVID-19 screening questions.
  5. Select "Any Location."
  6. Select "Any Provider."
  7. Select the date that corresponds with the first date of your desired presentation, and click the orange time button underneath.  You can find this information in the class advertisement.
  8. In the "What do you want to discuss during this visit," enter the name of the presentation you are registering for.
  9. Verify your appointment details and click “Schedule."
  10. A link for joining the seminar will be provided prior to the date of the event. Links will be sent through MyChart on the Premise patient portal.

If you scheduled an appointment through the CareHere app or patient portal, go to mypremisehealth.com to verify that your appointment transferred to the My Premise Health patient portal.  If it didn't, please call Premise Health at (855) 200-6822.

 

At Your Appointment

  • Bring a valid photo ID and your insurance card to your appointment.
  • Plan to arrive 5 to 10 minutes before your appointment time to complete the new check-in process.
  • If you are running late, there is a 5-minute grace period for a 15-minute appointment and a 10-minute grace period for a 30+ minute appointment. 

How to Cancel an Appointment

  1. Call Premise Health at (855) 200-6822 or login to mypremisehealth.com.
  2. The home page will show your upcoming appointments. Click “View Details” next to the appointment you wish to cancel or reschedule. 
  3. Reschedule or cancel your appointment, as applicable.
