Health Coaching

State Plan members residing in health center areas are invited to work with certified health and wellness coaches at no cost.

State Plan members who reside outside of health center areas are required to have one annual in-person appointment at a health center prior to meeting with a health coach, at no cost to you. The in-person appointment can be for any reason such as a health screening or an acute care visit.
